Resumen
La aftosis compleja o bipolar comprende la manifestación conjunta de úlceras orales y genitales. Se comunican dos casos: el primero corresponde a una mujer de 29 años de edad con antecedente de úlceras orales y genitales recurrentes, dolorosas, sin comorbilidades asociadas, quien mostró alivio importante con corticoesteroides tópicos y sistémicos. El segundo caso corresponde a otra mujer de 47 años de edad, con antecedente de úlceras en la mucosa oral y genital, dolorosas e incapacitantes, quien había sido sometida a trasplante renal por insuficiencia renal terminal secundaria a síndrome de Goodpasture, con tratamiento inmunosupresor con prednisona, micofenolato de mofetilo y tacrolimus. Este artículo discute las causas, el origen probable y la labor diagnóstica enfocada en descartar enfermedades predisponentes a la aparición de aftas bipolares. Abstract
Complex aphthosis or bipolar aphthosis implies simultaneous presentation of oral and genital ulcers. We present two cases. The first one corresponds to a 29 year-old female who presented with recurrent and painful oral and vulvar ulcers. She had no other known comorbidities and showed marked improvement with topical and systemic steroid treatment. The second case refers to a 47 year-old female with history of bipolar aphthosis that was also incapacitating. She had undergone renal transplantation because of renal failure due to Goodpasture syndrome and was on immunosuppressive medication including prednisone, mycophenolate mofetil and tacrolimus. On this report we emphasize the causes, probable etiologies and diagnostic workup in search of predisposing factors of this disease.
